Support coordination Level 1 and Level 2

Level 1: Support Connection

Purpose:

To increase a participant's capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in the community.
Scope: Support Connection helps participants connect with both informal, community, and funded supports. It focuses on building the participant’s ability to manage their own NDIS plan.


Activities:

This may include assistance with finding and connecting with service providers, understanding service agreements, and developing participant's skills to coordinate their own supports.

Level 2: Coordination of Supports

Purpose:

To build the participant’s ability to design and build supports to achieve their goals.

Scope:

Coordination of Supports involves a higher level of support compared to Support Connection. It aims to support participants in understanding and implementing their NDIS plan, and in building their ability to exercise choice and control.

​

Activities:

This includes more complex tasks such as resolving service delivery issues, managing multiple supports and services, supporting the participant to understand their plan, exploring housing options, developing resilience in the participant’s own network, and coordinating service delivery to avoid duplication.
In summary, while Level 1 focuses on helping participants connect with supports and manage basic aspects of their NDIS plan, Level 2 offers more intensive assistance to help participants navigate and implement their plan effectively and build their capacity to manage their supports independently
